We are so glad you are here! We have made it our priority to bring excellent care to the Star community and surrounding areas. We believe healthcare should be a partnership between you, the patient and your provider. Because of this we have streamlined the visit process for you to have more one on one time to achieve a sense of education and overall wellness with your health journey. To become a New Patient:

To establish care and schedule an appointment please call our office at 208-286-9471.

To transfer records to Owyhee Family Medicine:

If you wish to transfer your care to Star Family Health and Wellness, please download and complete our Authorization to Release Records form. Once completed, please submit the request form to the office where you are currently being seen.

Health History & Consent Forms: 

Forms are available on the Patient Portal after setting up a patient account.  You can complete your health history and sign the consent forms, from the comfort of your home.  If you are unable to complete them online prior to your appointment, it is required that you arrive 25 minutes earlier than your scheduled appointment time.
